This week’s Issue | 22/10/2012 Last week we began the facilitation of a full blown FMEA (Failure Mode Evaluation Analysis) exercise in Saskatoon.  The flow of a robust FMEA consists of the following: • Creation of the Critical To Trees ...  This creates the voice of the customer • SIPOC • Detailed Process Flow Chart ... which needs to include the input and outputs of each process step • Cause and Effect Diagram • Cause and Effect Matrix • FMEA What really struck us ... if an organization really reflects on the
